DR. NELSON J. OPPERMANN, DDS, MS

Growth of craniofacial structures depends - in some extent - to the adaptation to the occlusal plane and function, especially the mandible. Studies from Prof. Sato's and Slavicek's groups have shown that the inclination of the posterior occlusal plane (POP) is determinant in the adaptation and position of the mandible. When the inclination of the POP is Postero-Superior, the mandible tendency is not to adapt anteriorly, and inversely, when the POP is too low, the mandible tends to grow forward. The development of skeletal malocclusion, such as Class II convex, class III concave and mandibular lateral deviation (MLD), are determined by their particular occlusal plane configurations and how the mandible and other structures adapt. The Canted Frontal Oclusal plane is associated with asymmetries and mandibular lateral deviations which are frequently treated by combining Orthodontics with orthognatic surgery. New possibilities are emerging by correcting the 3D Oclusal Plane and the craneo mandibular system has shown some interesting adaptive capacity. Applying some bio-mechanical principles on the occlusal plane we can correct some mandibular lateral deviations (MLD) and avoid many surgeries. The aim of this workshop is to present the main principles in the diagnosis, planning and treatment of MLD cases.

Introduction to the course : A temporary anchorage device (TAD) is a device that is temporarily fi xed to bone for the purpose of enhancing orthodontic anchorage either by supporting the teeth of the reactive unit or by obviating the need for the reactive unit altogether, and is subsequently removed after use This appliance signifi cantly enhances treatment capabilities and can be extremely effective in reducing treatment time, surgeries, and extractions. Appliances can be inserted chairside and loaded immediately. Recently, a number of case reports have appeared in the orthodontic literature documenting the possibility of overcoming anchorage limitations via tad’s offering almost limitless comfort in complicated treatment. This course describes the development of skeletal anchorage and provides an overview of the use of implants for orthodontic anchorage affording serenity in complex cases, and offering exceptional results for your patient. NEW 23Course program The participant understands effciency of the implant as orthodontics anchorage and ,this way , can improve the results of its treatments in various clinical situations. Regarding the placement of the implants, various clinical situations are managed/ • Treatment of class II with vestibular MIA in maxillary in non-extraction cases • Open bites with palatal MIA to intrude molars • Anterior placement of MIA to intrude the incisors • MIA placements to upright the molars • MIA placements for molar protractions • MIA placements for extraction cases in bi-protrusion cases Furthermore,we will study the placement methods according to bone thickness and we will consider the factors that improve your success rate • The proper amount of force • Methods of loading • Limit of age • Minimizing impingement of tissue • Removing fractured MIA • Factors affecting the success rate • Management of mobility • Management of occlusal load

The Aim is to illustrate features and advantages of complete 3D management in orthodontics using CT Cone beam, intraoral and face scanner. New generation of CT Cone beam for dentistry allows to visualize and analyze 3D images of jaws, teeth and soft tissues of the patient. After a single exposure it is possible to match the DICOM file obtaining a 3D cephalometric tracing and a superimposition on stl file from intraoral scanner to perform a customized setup of the malocclusion. In this way it is possible to simulate not only the movements of the crowns but especially of the roots, with respect of the real bone limits of the patient. Matching these data with the soft tissue of the patient from the face scanner we are able to have the entire virtual patient, in order to better analyze the case, to make a prevision of the expected results or to perform growth prediction without any other irradiation. On the digital setup of the malocclusion can be virtually positioned the brackets, in order to make a customized transfer system for indirect bonding, that is very important especially in lingual orthodontics. The intraoral scanner allows to obtain solid casts of the dental arches of the patients, by using a cad-cam technology and stereo-lithographic machines. With specific procedures and dedicated software it is also possible to design and produce different fixed orthodontic appliances, without having any stone model, in a full digital workflow. The new possibilities offered by CT Cone beam and digital scanner allow to make a full and accurate 3D planning for orthodontic treatments, from diagnosis to indirect bonding, using a single x-ray exposure for the patients. This workshop will give the background of orthodontic digital technology and provide the current "state-of-the-art" of 3D orthodontics. Learning objectives: 1. Full and accurate diagnosis for orthodontic treatments 2. 3D planning for orthodontic treatments 3. Project of custom digital orthodontic devices 4. Different way to effective cooperate with dental Labs

How in the world can we incorporate the 10 principles of the Bioprogressive philosophy first published in 1977 into a modern day practice? It is very easy, in fact! The “godfathers” of the Bioprogressive movement were years ahead of their times. They taught that delegation, incorporating technology, and creating systems to improve treatment quality, stability, and efficiency was the key to the future. In this program I will begin by reviewing those principles first taught in 1977 and take you into a leading edge approach to the practice of orthodontics.

Early orthodontic treatment is a highly discussed topic in the orthodontic field. Very often there are significant variants in the definition of a treatment composed only by one phase or a treatment composed by two phases During the presentation, the opportunity of treatment in early mixed dentition will be analyzed. Several alterations of the going on occlusion will be presented, in particular, in the vertical component of the growth. The proposed therapeutic approach is based on the use of silicone made Eruption Guidance Appliances which, using the potential power of eruption of teeth and piloting them inside the mouth, give us a very interesting therapeutic opportunity, particularly in the early mixed dentition.

Class II malocclusion is one of the most common condition in the Caucasian race. Traditionally it is suggested that the correction should be started at the peak of the growth. This kind of approach does not consider the total amount of the growth, the psychologically impact of the class II profile and the risk of the incisal trauma on the permanent dentition. On the other hand, the Bioprogressive approach suggests treating as soon as possible, during the deciduous dentition, in order to allow the establishment of a normal function and the physiologic development of cranium-mandibular system, individualizing the treatments in a logical sequence easily achievable by every clinician. The speaker will show these concepts by the use of evidence based literature and clinical cases.

In the past, the TMJ was often considered a grey area and few centers were engaged in surgical treatment of its pathology. With the development of new diagnostic imaging possibilities and new surgical techniques, surgery of the TMJ, in cases of acute injury or chronic internal derangement, is becoming a standard procedure at many clinics. In the presentation, conditions leading to TMJ damage will be discussed, different modalities of diagnostic imaging presented, and surgical approaches and procedures thoroughly explained.

In Zerobase orthodontic philosophy, priority is given to the diagnosis and at the determination of treatment’s objectives. The notion of degree of difficulty is resulting from this evaluation and will guide practitioner choices. Clinical examination and visual examination of X-rays could orient us to a surgical decision. The 3D surgical visual treatment planning will help us in our decision-making and in the determination of three-dimensional treatment objectives for the teeth, bones pieces, and to reach optimum functional and cosmetic results for each patient. It will also help us for the communication towards the patient and the surgeon. The surgical protocol implemented should allow to reach an ideal positioning of the teeth within each arch and ensure the arch transverse coordination. The mounting on SAM articulator will allow the surgical simulations, which are necessary to make splints. We should not try to avoid surgery (to look for alveolar compensations can represent an important functional risk), but evaluate the needs for surgical intervention, always in the respect of benefits / risks for the patient. It is particularly true in treatment of sleep disordered breathing.

Decisions in our field in some degree still are dominated by many traditional paradigms, such as: genetics dominance vs environment, extractions or not extractions, two phase or one phase treatment, condilar growth, stability, cervical vs high pull headgear, etc. Components involved in some malocclusions are not limited to teeth, many structures of the cranio-mandibular system (CMS) may be affected, in such cases orthognatic surgery is one very common approach. This dissertation will show new potentials and new limits of orthodontics that in some way challenge traditional paradigms. Treatments based on 3D occlusal plane control are able to induce remarkable transformation or adaptation of the CMS allowing to treat very complex malocclusions, such as severe openbites, class III open bites and mandibular lateral deviations cases. Moreover, in growing individuals, growth control through occlusal plane alteration can transform its final outcome. Presenter will show treatment cases and recent literature evidence.

The history of orthodontics has evolved through numerous, philosophies, concepts and techniques. However, to understand and treat craniofacial dysfunctions, we must understand posterior discrepancy and occlusion of individual phenotypes in human today, and the influence by different evolutionary and development process. The sophistication of the human craniofacial system is not a simply result of a steady evolution. These create an important task for human. Philosophy of Malocclusion etiology and development is necessary. Usually, orthodontists treat malocclusions only based on symptoms. “Begin with the end in mind”. Orthodontic occlusal construction with occlusal plane is the key point in occlusal therapy. In these cases, reports treatment, the clinical procedures applied were with the use of multiloop arch wire (MEAW), in different types of malocclusion. It is, however, important to remember that were insertion of the MEAW does not guarantee treatment success. MEAW therapy requires a keen sense of judgment in diagnosis and treatment planning. The aim of the orthodontic treatment is first of all the knowledge of occlusion dysfunction, occlusal concept, and his success depends on a vast scientific knowledge, intuitive skills and a strong artistic interpretation.
